Meet The Founder

Hello, I’m Ruchi Sharma, founder of Well Connect LLC.

As both a parent with personal Self-Determination Program experience and an Independent Facilitator, I understand the emotional and practical challenges families often face while navigating services and supports. My mission is to help families feel heard, organized, and supported while creating individualized plans focused on long-term success, independence, and quality of life.

I work closely with families to provide support with Person-Centered Planning, budget and spending plan guidance, Independent Facilitator services, service coordination assistance, and ongoing advocacy throughout the SDP journey.

At Well Connect LLC, every family receives compassionate support, personalized attention, and organized guidance every step of the way..

A few fun facts about me — I live with my family and our dog Bruno, who brings so much joy and energy into our home. I love animals, enjoy painting in my free time, and genuinely enjoy connecting with and meeting new people. We begin by learning about your child, family priorities, current services, strengths, and areas where additional support may be needed. At Wellconnectllc, our founder Ruchi Sharma brings personal and professional experience to support families through the Self-Determination journey. We provide compassionate facilitation to help clients achieve their long-term goals with confidence.

Person-Centered Planning

Together, we create a meaningful and individualized Person-Centered Plan focused on goals, independence, daily living, community participation, and long-term success.

Budget & Spending Plan Support

We help families understand available funding categories, service options, spending plans, and SDP budget development.

Transition & Coordination

Support with onboarding, vendor coordination, documentation, communication, and transition into the Self-Determination Program.
